ORIGINAL ARTWORK AND COMMISSION ORDER FORM

If you'd like to place an order for artwork:

• Please fill out this form with the appropriate quantities and total.
  • Shipping artwork can be expensive—usually no less than $50.00 per piece, and often up to $100.00. This can't be avoided since the pieces are framed with glass, and they are original artwork and must be insured.
  • In order to get an accurate estimate, I need an exact street address including town name and zip. I can't obtain an estimate without this.
• E-mail this order to me and tell me whether you'd like to pay with check, money order or credit card.
• I will check it over, total your order, get a quote on shipping to your address and respond via e-mail.
  • If you are paying by check or money order, I will e-mail my address.
  • If you are paying by credit card I will request your phone number so that I can call you for your card number (it's currently the most secure way).
• I will ship your order as soon as I receive payment and the check or credit card transaction has been approved.
• Once the transaction has been completed, I destroy all record of your credit card number, and I will never, ever sell your phone number or shipping or e-mail address to anyone.

If you live in the state of Pennsylvania, I need to charge 7% sales tax. Sorry. I don't like it either.
